How to Appeal a Kaiser Permanente Denial

How do I appeal a Kaiser Permanente denial?

With Kaiser Permanente, care delivery, billing, and health-plan decisions may feel closely connected, but the problem still determines the route. A concern about an appointment is different from challenging a denied service or reimbursement request.

General U.S. guidance. Your plan documents and denial notice explain the process that applies to your case. Medicare, Medicaid, and other programs may use different rules. This is not medical or legal advice, and no result is guaranteed.

Choose your region and plan

Kaiser’s website uses regional resources. Confirm the region and whether your plan is commercial, Medicare, or Medicaid. Use the Evidence of Coverage and denial instructions for your product. A Southern California Medicare page should not be treated as a national commercial appeal rule.

Clarify what you want reviewed

For an incorrect charge, ask billing to investigate the account. For a denied benefit or reimbursement request, state that you want review of the coverage decision. For service or access concerns, ask Member Services which grievance process applies. One conversation may raise more than one issue, so ask which requests were formally recorded.

Use the official regional support resources

Kaiser provides regional support and complaint resources, with additional Medicare instructions. Ask Member Services for the applicable form, channel, deadline, and representative requirements. Do not assume a message to your clinician is a formal coverage appeal.

For Medicare Advantage decisions

Kaiser’s 2026 Southern California Medicare resource describes a 65-day Level 1 appeal window from the notice date, and refers members to Chapter 9 of the Evidence of Coverage. Follow your own notice, especially for discharge or termination of ongoing services, where fast-appeal deadlines may differ.

Prepare records and confirm the case

Attach the denial, service or authorization details, and records supporting the specific disagreement. Ask for receipt and the expected review date. If clinical urgency is involved, request the appropriate expedited process with clinician support. Read the decision for the next review route, and keep any billing hold separate from the appeal.

Which plan rules apply?

For commercial coverage, check the policy and state-specific instructions. A self-funded employer plan can use different oversight and appeal rules even when an insurer administers claims. Medicare and Medicaid use program-specific notices and review routes. Confirm the applicable deadline, whether a correction counts as an appeal, and where to escalate; do not substitute a deadline from a different plan.

Sources and further help

These sources provide general guidance. For a deadline or requirement that applies to you, follow your plan’s notice or the appropriate program’s current instructions.

Insurer websites may ask you to select your state or sign in. If a page will not open, use the member services number on your insurance card and ask for the current appeal instructions for your plan.
