How to Appeal an Anthem Denial

How do I appeal an Anthem denial?

Anthem’s forms and appeal routes vary by state and coverage product. A California individual-plan page is useful for that coverage, but it is not a national instruction sheet for employer, Medicaid, and Medicare members.

General U.S. guidance. Your plan documents and denial notice explain the process that applies to your case. Medicare, Medicaid, and other programs may use different rules. This is not medical or legal advice, and no result is guaranteed.

Find the exact plan and state

Use the ID card and denial letter to identify the Anthem entity and product. Enter the correct state on Anthem’s official site and check the member complaints, grievances, and appeals resources. If a medical group or another organization issued the denial, ask Anthem where the member appeal belongs.

Treat state examples as examples

Anthem’s California individual and family resource describes a grievance and appeal process and a 180-calendar-day filing period. Other products can differ. Use the current notice for your claim and ask the member number on your card to confirm the correct deadline, form, and submission channel.

Explain whether you dispute coverage or service

State clearly that you want review of a specific denied claim or authorization if that is your goal. A complaint about customer service is a different concern. Attach the denial, service dates, relevant plan language, and the records that answer the insurer’s explanation.

For employer or public-program coverage

Ask whether an employer plan is self-funded and who controls the benefit decision. Medicare and Medicaid products have separate appeal and grievance instructions. Do not send a Medicare request using a general individual-plan form simply because the Anthem name appears on both.

Confirm the appeal and monitor billing

Request a receipt, case reference, and expected decision date. Ask the treating clinician about urgent review where medically needed. Separately ask the provider for a billing hold; the appeal alone does not establish a hold. Read a final denial for further review and regulator information specific to your plan.

Which plan rules apply?

For commercial coverage, check the policy and state-specific instructions. A self-funded employer plan can use different oversight and appeal rules even when an insurer administers claims. Medicare and Medicaid use program-specific notices and review routes. Confirm the applicable deadline, whether a correction counts as an appeal, and where to escalate; do not substitute a deadline from a different plan.

Sources and further help

These sources provide general guidance. For a deadline or requirement that applies to you, follow your plan’s notice or the appropriate program’s current instructions.

Insurer websites may ask you to select your state or sign in. If a page will not open, use the member services number on your insurance card and ask for the current appeal instructions for your plan.
